RESUMO Objetivo Analisar a correlação entre os eixos estruturantes do psiquismo no roteiro IRDI (Indicadores de Referência ao Desenvolvimento Infantil) e na aquisição da linguagem por meio dos Sinais Enunciativos de Aquisição da Linguagem (SEAL) quanto ao papel do bebê e da mãe nas protoconversações iniciais. Método A amostra contou com 77 bebês, que concluíram todas as avaliações no IRDI e no SEAL. Os resultados obtidos por meio desses dois instrumentos foram analisados por meio do coeficiente de Spearman e do teste de U de Mann-Whitney, considerando valor de p<= 0,05. Resultados Identificou-se correlação significativa entre as alterações nos indicadores relacionados à função materna (suposição de sujeito, estabelecimento da demanda e alternância presença/ausência) e as alterações nos sinais enunciativos dos bebês e das mães. Não houve correlação significativa na relação isolada do eixo função paterna e a presença de atraso na aquisição da linguagem. Conclusão Houve correlação significativa entre os eixos de função materna do roteiro IRDI e risco à linguagem pelo SEAL.

ABSTRACT Purpose To analyze the correlation between the psychism structuring pillar in the RIID (Reference Indicators of Infant Development) script and language acquisition through Enunciative Signs of Language Acquisition (ESLA) as to the role of the baby and the mother in early protoconversations. Methods The sample included 77 infants, who completed all the RIID and ESLA assessments. The results obtained from these two instruments were analyzed using Spearman's coefficient and the Mann-Whitney U test, considering a p-value < 0.05. Results A significant correlation was identified between changes in the indicators related to the maternal function (assumption of subject, establishment of demand and presence/absence alternation) and changes in the enunciative signals of babies and mothers. There was no significant correlation in the isolated relation of the paternal function pillar and the presence of delayed language acquisition. Conclusion There was a significant correlation between the pillars of maternal function of the RIID script and language risk by ESLA.

ABSTRACT Purpose To analyze the correlation between the results obtained on the SEAL and the Bayley III Scale and compare babies with and without delay in language acquisition at 24 months concerning the performance obtained by them and their mothers on the SEAL from 3 to 24 months. Methods The SEAL collection consists of 15-minute footages of 45 babies aged from 3 to 24 months old in interaction with their mothers, who were assessed by two trained speech therapists for the use of the SEAL. At 24 months, the 45 babies were assessed using the Bayley III Scale and the item language was selected to classify them with and without delay. These results were statistically analyzed through a Pearson’s correlation test and a Fisher's exact test. Results In average, eighteen signs of typical development as we obtained, while a mean of 12 delay signs were found. By comparing the presence and absence of signs between the groups with and without delay in language acquisition, eight signs from the baby and one from the mother differed statistically in the sample. The analysis using the SEAL for cases of delay showed that the maternal factor was as important as the infant factor to understand the babies’ language functioning. Conclusion There was a significant correlation between the SEAL performance from 3 to 24 months and the language outcome at 24 months assessed by the Bayley III Scale in this sample.

RESUMO Objetivo Analisar a correlação entre resultados obtidos no Sinais Enunciativos de Aquisição da Linguagem (SEAL) e na Escala Bayley III e comparar bebês com e sem atraso na aquisição da linguagem aos 24 meses no desempenho obtido por ele e sua mãe no SEAL dos 3 aos 24 meses. Método A coleta do SEAL constou de filmagens de 45 bebês, realizadas nas faixas etárias de 3 a 24 meses em interação com suas mães, com duração de 15 minutos, que foram avaliados por duas fonoaudiólogas treinadas no uso do SEAL. Aos 24 meses, os 45 bebês foram avaliados pela Escala Bayley III e selecionado o item linguagem para classificá-los com e sem atraso. Sobre tais resultados realizaram-se as análises estatísticas com o teste de correlação de Pearson e o teste exato de Fisher. Resultados Obtiveram-se as médias de sinais no desenvolvimento típico que foi 18 sinais e, em casos de atraso, a média foi de 12 sinais. Na comparação da relação de presença e ausência dos sinais entre os grupos com e sem atraso na aquisição da linguagem, oito sinais do bebê e um da mãe diferiram estatisticamente na amostra. O fator materno apresentou-se tão importante quanto o infantil na compreensão do funcionamento de linguagem dos bebês na análise realizada com o SEAL nos casos de atraso. Conclusão Houve correlação significativa entre o desempenho no SEAL entre 3 e 24 meses e o desfecho de linguagem aos 24 meses avaliado pela Escala Bayley III nesta amostra.

RESUMO Objetivo Analisar a validade de conteúdo e de construto de sinais enunciativos de aquisição da linguagem para crianças de 13 a 24 meses. Método Os sinais criados foram submetidos à avaliação de clareza e pertinência por seis juízes experts em aquisição da linguagem na perspectiva enunciativa. A partir do seu julgamento foi produzida uma versão experimental que foi aplicada na análise de vídeos de interações mãe-bebê/examinador, com duração de 15 minutos, de 77 díades mãe-bebê na faixa etária de 13 a 18 meses e 89 díades na faixa etária de 19 a 24 meses. A validação de fidedignidade e consistência interna foi realizada pela análise de 10% da mostra por dois juízes fonoaudiólogos. A validação do construto foi realiza pela análise fatorial efetivada sobre a amostra total. Os dados foram analisados no Statistica 9.1 e PASW 17.0. Resultados Todos os sinais foram considerados claros e pertinentes pelos juízes experts. A análise de fidedignidade apontou a concordância quase perfeita (0,8 ≤ Kappa ≥ 1,0) para a maior parte dos sinais. A consistência interna para a Fase 3 apresentou alpha=0,771 considerada alta e Fase 4 apresentou alpha=0,917 limítrofe a muita alta. A análise fatorial da fase 3 revelou 2 fatores, explicando 59,1% da variância total e a fase 4 revelou 1 fator, explicando 75,7%. Conclusão Observou-se validade de conteúdo e de construto para cinco dos doze sinais na Fase 3 e para todos sinais na fase 4.

ABSTRACT Purpose To analyze the content and construct validity of enunciative signs of language acquisition for children aged 13 to 24 months. Methods The signs created were subjected to an assessment of clarity and relevance by six expert judges in language acquisition from an enunciative perspective. Based on their judgment, an experimental version was produced that was applied to the analysis of videos of mother-baby / examiner interactions, lasting 15 minutes, of 77 mother-baby dyads aged 13 to 18 months and 89 dyads in the age range aged between 19 and 24 months. The validity of reliability and internal consistency was performed by analyzing 10% of the sample by two speech therapists. The construct validation was carried out by the factorial analysis carried out on the total sample. The data were analyzed in Statistica 9.1 and PASW 17.0. Results All signs were considered clear and relevant by the expert judges. The reliability analysis showed an almost perfect agreement (0.8 ≤ Kappa ≥ 1.0) for most signs. The internal consistency for Phase 3 showed alpha = 0.771 considered high and Phase 4 presented alpha = 0.917 bordering on very high. The factor analysis of phase 3 revealed 2 factors, explaining 59.1% of the total variance and phase 4 revealed 1 factor, explaining 75.7%. Conclusion Content and construct validity were observed for five of the twelve signs in Phase 3 and for all signs in Phase 4.

ABSTRACT Purpose: to analyze the use of digital resources by Brazilian phonoaudiologists before and during the period of the COVID-19 pandemic. Methods: cross-sectional study carried out with a representative sample of Brazilian phonoaudiologists, with at least one year of practical experience. An online questionnaire consisting of 28 questions divided into eight sections was prepared. Data were analyzed and are reported as absolute and relative frequency regarding the variables: respondent’s age, highest degree and years of experience; weekly working hours; region of the country; type of service and area of expertise; use of digital means before and during the pandemic and willingness to use the same kind of resource after the pandemic. Results: telehealth technologies were used by 90% of phonoaudiologists in Brazil since before the COVID-19 pandemic. A change in the profile of technologies used was observed: after the pandemic, videoconferences gained more use in contact with patients compared to the pre-pandemic period even though contact via mobile phone and instant messaging applications are the most used, both before and during the pandemic. Conclusion: the study's findings demonstrate that 90% of phonoaudiologists who answered the questionnaire in Brazil used and still make use of telehealth technologies to contact patients.

Resumo Objetivo Analisar a relação entre seletividade alimentar e a disfunção do processamento sensorial em criança com Transtorno do Espectro Autista (TEA) e acompanhar sua evolução com abordagem terapêutica de intervenção sensorial. Método Trata-se de uma pesquisa qualitativa a partir de um estudo de caso, com amostra de conveniência de um menino de cinco anos com diagnóstico de TEA e seletividade alimentar, acompanhado durante um ano e cinco meses. Foi utilizado neste estudo de caso o Protocolo Perfil Sensorial – Questionário para os Pais – 3 a 10 anos e o roteiro sobre a alimentação. Resultados Foi identificada alteração significativa no Perfil Sensorial, principalmente nos sistemas que estão relacionados com a alimentação, confirmando as dificuldades sensoriais de crianças com TEA e sua interface com seletividade alimentar. O tratamento de terapia ocupacional com abordagem de integração sensorial obteve resultados favoráveis na aceitação dos alimentos e diminuição da seletividade. Conclusão Alterações no perfil sensorial estiveram relacionadas com a dificuldade alimentar, evidenciando que a seletividade no caso estudado tinha origem sensorial superada com terapia de integração sensorial.

Abstract Objective To analyze the relationship between food selectivity and sensory processing dysfunction in children with Autism Spectrum Disorder (ASD) as well as monitor their evolution with a therapeutic approach to sensory intervention. Method This is a qualitative research based on a case study, with a convenience sample of a five-year-old boy diagnosed with ASD and food selectivity, who was followed for one year and five months. In this case study, the Sensory Profile Protocol – Questionnaire for Parents – 3 to 10 years and the script on nutrition was used. Results A significant change in the Sensory Profile was identified, especially in systems that are related to food, confirming the sensory difficulties of children with ASD and its interface with food selectivity. The occupational therapy treatment with a sensory integration approach obtained favorable results in terms of food acceptance and decreased selectivity. Conclusion Changes in the sensory profile were related to eating difficulties, showing that the selectivity in the case studied had sensory origin, which was overcome with sensory integration therapy.

RESUMO Objetivo Investigar a associação de fatores sociodemográficos, obstétricos e psíquicos com atraso no desenvolvimento motor fino e grosso, em crianças nascidas a termo e prematuras. Método Os bebês nascidos a termo e pré-termo foram avaliados pelo teste Denver II para motricidade fina e grossa em três fases: 165 crianças na fase 1 (3 meses e 1 dia a 4 meses e 29 dias), 130 crianças na fase 2 (8 meses e 1 dias a 9 meses e 29 dias) e 102 crianças na fase 3 (11 meses e 1 dia a 12 meses e 29 dias). Os dados sociodemográficos, obstétricos e psicossociais foram obtidos em entrevista inicial e o risco psíquico foi avaliado pelos indicadores clínicos de risco ao desenvolvimento infantil e pelos sinais PREAUT. A análise estatística foi realizada por meio de regressão logística. Resultados Os fatores significativos na associação com atraso motor fino e grosseiro foram: histórico gestacional e obstétrico materno (gravidez planejada, tipo de parto, número de consultas pré-natais, uso de medicamentos e intercorrência gestacional), características e riscos biológicos do bebê (sexo, ventilação mecânica, dificuldade de alimentação), fatores sociodemográficos (carreira e nível de escolaridade materna, número de filhos e pessoas na casa) e questões psicossociais relacionadas à participação rotineira da família e presença de risco psíquico. Conclusão Houve associação significativa entre atraso no desenvolvimento motor dos lactentes, variáveis ambientais e biológicas, com ênfase no risco psíquico.

ABSTRACT Purpose To investigate the association of sociodemographic, obstetrical and psychosocial factors with fine and gross motor developmental delay in preterm and term infants, in the age group of three months and one day to twelve months and twenty-nine days. Methods The term and preterm infants were evaluated by the Denver II Test for fine and gross motor skills, investigated in three phases: 165 infants in phase 1 (3 months and 1 day to 4 months and 29 days), 130 infants in stage 2 (8 months and 1 day to 9 months and 29 days) and 102 infants in phase 3 (11 months and 1 day to 12 months and 29 days). Sociodemographic, obstetrical and psychosocial data were obtained through an initial interview with family members and the psychic risk assessment through the Child Development Risk Indicators and PREAUT protocols. Statistical analysis was performed using the logistic regression model. Results Significant factors in the association with fine and gross motor delay were: maternal gestational and obstetric history (planned pregnancy, type of delivery, number of prenatal consultations, use of medication and gestational intercurrence), features and biological risks of the baby (gender, mechanical ventilation, feeding difficulty), sociodemographic factors (career and level of maternal schooling, number of children and people in the house) and psychosocial issues concerning to the family routine participation and presence of psychic risk. Conclusion There was a significant association between motor development delay of the infants, environmental and biological variables, with emphasis on psychic risk.

RESUMO Verificar a relação entre a avaliação dos movimentos gerais (General Movements Assessment - GMA) com as variáveis obstétricas (aleitamento materno, intercorrência na gestação, medicação na gestação, álcool na gestação, fumo na gestação, intercorrência ao nascer, necessidade de internação em UTI neonatal e necessidade de ventilação mecânica), a presença de risco psíquico e o desfecho no desenvolvimento da linguagem, cognitivo e motor aos 18 e 24 meses. A amostra foi composta por 42 bebês, que foram filmados até a faixa etária de quatro meses, em movimentação espontânea por 15 minutos. Os movimentos gerais foram avaliados por vídeos usando a avaliação qualitativa de Prechtl e classificados como normais ou anormais dependendo da presença de fluência, complexidade e variabilidade. Os dados foram analisados estatisticamente em sua relação com variáveis obstétricas e com a presença de risco psíquico, avaliada por meio dos Sinais PREAUT, dos indicadores clínicos de risco ao desenvolvimento e do M-CHAT. Verificou-se que não houve associação entre o método GMA e as variáveis analisadas. Acredita-se que, pelo fato de a amostra ter sido composta, em sua maioria, por bebês nascidos a termo ou prematuros tardios sem intercorrências e de a avaliação ter sido de forma transversal, em um único momento, não foi possível analisar se os movimentos avaliados como anormais foram ou não transitórios. A alteração dos movimentos por meio do método Prechtl não apresentou associação com as variáveis analisadas na amostra de bebês prematuros tardios e nascidos a termo.

ABSTRACT To verify the relationship between the general movements assessment (GMA) and obstetric variables (maternal breastfeeding, pregnancy complications, gestational medication, alcohol consumption during gestation, smoking during gestation, intercurrence at birth, need for neonatal ICU admission, and need for mechanical ventilation), the presence of psychological risk and the outcome in language, cognitive, and motor development at 18 and 24 months. In total, 42 infants composed the sample and were filmed until the age of four months, in spontaneous movement for 15 minutes. The general movements were evaluated by videos using the qualitative evaluation of Prechtl and they were classified as normal or abnormal depending on the presence of fluency, complexity, and variability. The data were statistically analyzed regarding obstetric variables and the presence of psychological risk evaluated by the PREAUT signs, clinical indicators of developmental risk and M-CHAT. It was verified that there was no association between the GMA method and the analyzed variables. It is believed that because the sample was mostly composed of full-term infants or late premature infants without intercurrence, and the evaluation was transverse in a single moment, it was not possible to analyze whether the movements evaluated as abnormalities were or were not transient. The change of movements by the Prechtl method was not associated with the variables analyzed in the sample of late and term premature infants.

RESUMEN Este estudio tiene como objetivo verificar la relación entre la evaluación de los movimientos generales (GMA) con las variables obstétricas (lactancia materna, complicaciones del embarazo, medicación durante el embarazo, alcohol en el embarazo, tabaquismo durante el embarazo, complicaciones al nacer, necesidad de ingreso en la UCI neonatal y necesidad de mecánica ventilatoria), la presencia de riesgo psicológico y el resultado en el desarrollo del lenguaje, cognitivo y motor en bebés entre los 18 y 24 meses. La muestra constó de 42 bebés y se filmaron sus movimientos espontáneos por 15 minutos hasta los cuatro meses de edad. Los movimientos generales se determinaron por la técnica de evaluación cualitativa de Prechtl, los cuales se clasificaron como normales o anormales según la presencia de fluidez, complejidad y variabilidad. Los datos se analizaron estadísticamente con relación a las variables obstétricas y la presencia de riesgo psíquico, evaluadas por los Señales PREAUT, los indicadores clínicos de riesgo al desarrollo y el M-CHAT. Se encontró que no hubo asociación entre el método GMA y las variables analizadas. Debido a que la muestra estuvo compuesta mayoritariamente por bebés nacidos a término o prematuros tardíos sin complicaciones y que la evaluación fue transversal, en un solo momento, no fue posible analizar si los movimientos evaluados como anormales fueron transitorios o no. Los cambios en los movimientos mediante el método de Prechtl no se asociaron con las variables analizadas en la muestra de prematuros tardíos y nacidos a término.

Abstract Introduction Cortical auditory evoked potentials (CAEPs) can be used to evaluate both peripheral and cortical components of auditory function, and contribute to the assessment of functional sensitivity and auditory thresholds, especially in neonates and infants. Auditory evoked potentials reflect auditory maturity and precede the acquisition of more complex auditory and cognitive skills, and are therefore crucial for speech and language development. Objective The aim of the present study was to determine the presence, latency and amplitude of CAEP components in response to verbal stimuli in children aged 2 years old. Methods The sample consisted of 19 subjects, 10 of whom were male while 9 were female. All of the participants were 24 months old at the time of assessment. Results A total of 17 of the participants displayed all components of the CAEP. Additionally, no significant differences were observed between genders or ears in the present sample. The presence of all components of the CAEP in subjects aged 2 years old confirms the existence of a critical period for the maturation of auditory pathways in the first 2 years of life. Conclusion In the present study, in addition to the P1/N1 components, it was possible to observe the presence of the CAEP P2/N2 components in individuals aged 24 months, confirming the existence of a critical period for the maturation of the auditory pathways in the first 2 years of life.

The COVID-19 pandemic brought challenges and the need for adaptation on several fronts. Updating the ways of working is necessary and promoting education and care in the health sector in line with the 21st century is essential. In addition to undergraduate students training in Speech-Language Pathology (SLP) field, based on the necessary opportunities for practical experience with patients during undergraduation, a significant portion of the population is attended by school clinics linked to universities and training centers, which work in conjunction with public health services. In this text we will bring some reflections on the relevance of using telehealth technologies for speech therapists training, considering the articulation between national curricular guidelines, professional training and ethical aspects.

A pandemia da COVID-19 trouxe desafios e necessidade de adaptação em diversas frentes. A atualização das formas de trabalho é necessária e promover ensino e atendimento no setor saúde condizentes com o século XXI é fundamental. Não estamos apenas tratando da formação de graduandos na área de Fonoaudiologia, mas do atendimento clínico a uma parcela significativa da população que depende dos serviços de clínicas-escola vinculadas às universidades e centros de formação em nossa profissão.Neste texto traremos algumas reflexões sobre a relevância do uso das tecnologias de telessaúde para a formação de fonoaudiólogos, considerando a articulação entre as diretrizes curriculares nacionais, a formação profissional e aspectos éticos.

RESUMO Objetivo: Realizar a validação de construção dos Sinais Enunciativos de Aquisição da Linguagem para crianças de 3 a 12 meses. Método: A amostra contou com 94 díades de mãe-bebê para análise dos sinais da fase 1 (3 a 6 meses) e 61 díades para análise dos sinais da fase 2 (7 a 12 meses). A coleta de dados ocorreu por meio da análise da interação entre mães e bebês em situação lúdica gravada em vídeo nas fases previstas, com atribuição de valor do tipo presente ou ausente a cada sinal analisado em cada fase. Os dados foram organizados em planilha e convertidos para aplicação de software para análise factorial. Resultados: A análise factorial indicou a existência de dois fatores nomeados “da mãe” e “da criança” na análise da fase 1 dos sinais (explicando 71,9% da variação total), com três sinais relevantes para o fator “da criança” e um sinal relevante para o fator “da mãe”, com relação à fase 2 (explicando 74,4% da variação total), um sinal foi relevante para o fator “da criança” e um para o fator “da mãe”. Conclusão: A validação de construção demonstrou que um fator materno e um fator da criança foram hábeis para distinguir grupos de risco e não risco em ambas as fases analisadas, o que sugere que a ausência desses sinais pode indicar risco à aquisição da linguagem.

ABSTRACT Purpose: To perform the construct validation of enunciative signs of language acquisition for children aged 3 to 12 months. Method: The study sample consisted of 94 mother-infant dyads for the analysis of Phase 1 (3-6 months) signs and 61 dyads for the analysis of Phase 2 (7-12 months) signs. Data collection was performed through analysis of the interaction between mothers and infants in play situation recorded in 15-min videos in the predicted phases, with attribution of value types of present or absent to each sign analyzed. The collected data were organized on a spreadsheet and then converted to computer applications for factor analysis. Results: Factor analysis indicated the existence of two factors named “mother” and “infant” both for Phase 1 signs (explaining 71.9% of the variation) - with three signs relevant for the “infant” factor and one sign relevant for the “mother” factor, and for Phase 2 signs (explaining 74.4% of the variation) - with one sign relevant for the “infant” factor and one sign relevant for the “mother” factor. Conclusion: Construct validation showed that one “mother” factor and one “infant” factor were able to distinguish between at-risk and not-at-risk groups in both phases analyzed, which suggests that the absence of these signs may pose risks to language acquisition.

RESUMO Objetivo Comparar a frequência de indicadores de risco em bebês nascidos pré-termo e a termo; analisar as possíveis relações entre a presença de risco para perda auditiva com variáveis socioeconômicas, demográficas, obstétricas e risco à linguagem. Método Trata-se de um estudo de coorte longitudinal com amostra de 87 bebês. Foram coletados dados gestacionais, obstétricos e sociodemográficos das mães e dos bebês. A classificação socioeconômica das famílias foi analisada por meio do Critério de Classificação Econômica Brasil. O risco à linguagem foi avaliado por meio dos Sinais Enunciativos de Aquisição da Linguagem e Teste Denver II. Os dados foram analisados utilizando o programa STATISTICA 9.1, por meio dos Testes Quiquadrado e U de Mann-Whitney, Modelo de regressão linear simples e múltiplo. Resultados A permanência em UTI neonatal (65,52%), ototóxico (48,28%), ventilação mecânica (39,66%) e hiperbilirrubinemia (46,55%) foram os indicadores de risco mais frequentes na amostra. Considerando fatores socioeconômicos, demográficos e obstétricos, houve correlação entre pré-natal, idade gestacional, peso ao nascer e alimentação com o risco auditivo. A aquisição e desenvolvimento de linguagem mostrou significância estatística com a varicela, HIV, Apgar e peso >1500 gramas. Conclusão Os prematuros apresentaram maior frequência de indicadores de risco, comparados aos bebês a termo. Dos fatores ambientais, o pré-natal que interfere no desfecho da idade gestacional, peso ao nascer, apgar e presença de doenças infecciosas, além da alimentação, despontaram como significativos relacionados com o desenvolvimento da audição e a aquisição da linguagem. A prematuridade foi o fator biológico relevante relacionado ao risco auditivo e linguístico.

ABSTRACT Purpose To compare the frequency of risk indicators in preterm and full-term babies; to analyze the possible relationships among the presence of risk for hearing loss with language acquisition and socioeconomic, demographic and obstetric variables. Methods This is a longitudinal cohort study, with a sample of 87 babies. Gestational, obstetric and sociodemographic data were collected from mothers and babies. The socioeconomic classification status of the families were classified using the Brazilian Criteria for Economic Classification. The risk for language was assessed using the Language Acquisition Enunciation Signs and the Denver II test. The data were analyzed using the STATISTICA 9.1 software, using the chi-square and the Mann-Whitney U tests and simple and multiple linear regression models. Results Permanence in a neonatal intensive care (65.52%), ototoxic (48.28%), mechanical ventilation (39.66%) and hyperbilirubinemia (46.55%) were the more frequent risk indicators in the sample. Regarding socioeconomic, demographic and obstetric factors, there was a correlation among prenatal care, gestational age, birth weight, feeding with hearing risk. Acquisition and development of language showed statistical significance with varicella, HIV, Apgar score and birth weight >1500 grams. Conclusion Preterm babies showed higher frequency of risk indicators compared to full-term babies. Among environmental factors, prenatal care, which interferes in the outcome of gestational age, birth weight, Apgar score and presence of infectious diseases, as well as feeding, emerged as significant factors related to hearing and language acquisition. Prematurity was the relevant biological factor related to hearing and language risk.

Resumo Introdução: Atuais pesquisas sobre intervenção precoce (IP) em bebês com risco ao desenvolvimento ‒ entre os quais, o prematuro ‒ convidam terapeutas à construção de novos tratamentos clínicos. Objetivo: Analisar a eficácia de uma Hipótese de Funcionamento Psicomotor (HFP) para tratamento de bebê prematuro. Método: Tem cunho qualitativo, longitudinal, clínico quase experimental, de estudo de caso único. Amostra deu-se por conveniência, sendo um bebê prematuro, com risco psíquico e de desenvolvimento, identificado pelos protocolos PREAUT e IRDIS. Foi tratado durante três meses em IP por terapeuta ocupacional, a partir de HFP. O tratamento foi filmado e registrado em diário de campo. Posteriormente analisado através da comparação entre objetivos propostos e resultados alcançados, destacando os efeitos da HFP. Resultados: O caso confirma que a irregularidade psicomotora pode ser compreendida como sintoma psicomotor e sua origem pode ser interpretada a partir de uma HFP. Considera a irregularidade de forma singular, como anúncio de como a criança constrói seu Esquema (EC) e Imagem Corporal (IC), a partir do laço com seus pais. Confirmou-se a eficácia do tratamento a partir de uma HFP, pois o sintoma psicomotor desapareceu (EC), dando espaço para aquisições psicomotoras e cognitivas, e para a ressignificação dos investimentos da mãe em seu bebê (IC). O EC é tomado como funcionamento motor e cognitivo, que anuncia o funcionamento psíquico relacionado a IC em elaboração. Conclusão: A HFP interpreta o processo psicomotor do bebê supondo como está se constituindo, de modo a permitir uma IP que possa transformar o sintoma psicomotor em gesto estruturante.

Abstract Introduction: Ongoing studies about premature intervention (PI) on babies with developmental risk, like prematures, invite therapists to construct new clinical treatments. Objective: To analyze the effectiveness of a Hypothesis of Psychomotor Functioning (HPF) for treatment of the premature babies. Method: Qualitative, longitudinal, and clinical almost experimental single case study. We studied one premature baby, with developmental and psychic risk, identified by the PREAUT and IRDI protocols. The baby received three months of PI treatment by occupational therapy from the HPF. Treatment was filmed and registered in a field diary. We then compared the proposed objectives and the collected results, outlining the effects of the HPF. Results: The case confirms that the psychomotor irregularity may be understood as a psychomotor symptom and its source construed as from a HPF. The irregularity is considered in a singular way, as observation of how the child constructs its Body Scheme (BS) and Body Image (BI), starting from the bonds with parents. The treatment effectiveness was confirmed from a HPF, as the psychomotor symptom went away (BS), giving place for cognitive and psychomotor acquisitions, and to the maternal resignification of the investments in her baby (BI). The BS is taken as a cognitive and motor psychic functioning related to the BI in construction. Conclusion: The HPF interprets the baby psychomotor process based on its construction, allowing a PI that can transform a psychomotor symptom into a structuring motion.

Resumo Neste artigo, objetivou-se averiguar a relevância dos Determinantes Sociais da Saúde para o desenvolvimento do bebê com enfoque no desenvolvimento da linguagem. Além disso, realizou-se uma reflexão sobre as contribuições do estudo de casos como forma de adotar uma postura compreensiva e dinâmica na análise do desenvolvimento do bebê. Tais aspectos foram analisados com destaque para a relação de mutualidade mãe-bebê. Para tanto, foram acompanhadas duas díades mãe-bebê filmadas em três etapas da vida dos bebês: entre 20 e 30 dias; entre 80 e 90 dias; e entre 110 e 120 dias. As filmagens foram transcritas e analisadas partindo de fragmentos que ilustram a evolução longitudinal dos casos. Demonstrou-se que esse método de pesquisa permite compreender que os Determinantes Sociais da Saúde incidem de maneira acentuada sobre o desenvolvimento do bebê. Destaca-se que o enfoque na relação mãe-bebê de mutualidade amplia o escopo da atenção materno-infantil e permite verificar a sustentação do cuidado ao bebê por meio da articulação corpo/linguagem.

Abstract This article aims at verifying the relevance of the Social Determinants of Health for infant development focusing on language development. We analyzed the case study contributions as a way to adopt an understanding and dynamic posture in the analysis of infant’s development. These aspects were analyzed with emphasis on the mutual mother-infant relationship. For this purpose, we observed two mother-infant pairs filmed in three stages of the babies’ life: between 20 and 30 days, between 80 and 90 days, and between 110 and 120 days. The footage was transcribed and analyzed starting from fragments illustrating the longitudinal course of the cases, demonstrating that this method of research allows the understanding of how Health Social Determinants focus on the development of the baby. It stands out that the observation of the mother-infant relationship increases the scope of maternal-infant care and allows checking the support of infant care through the body/ language articulation.

Resumen El objetivo de este artículo consiste en averiguar la pertinencia de los Determinantes Sociales de la Salud para el desarrollo del bebé,con enfoque en el desarrollo del lenguaje. También se realizó una reflexión sobre las contribuciones del estudio de casos, como forma de adoptar una postura comprensiva y dinámica en el análisis del desarrollo del bebé, destacándose la relación mutualidad madre-bebé. Se observaron dos duplas de madre-bebé filmados en tres etapas diferentes de sus vidas: entre 20 y 30 días, entre 80 y 90 días y entre 110 y 120 días. Se transcribieron y analizaron las filmaciones a partir de fragmentos que ilustraban la evolución longitudinal de los casos, demostrando que este método de investigación permite comprender de qué forma los Determinantes Sociales de la Salud, inciden de manera acentuada en el desarrollo del bebé. Se destaca que el foco en la relación de mutualidad madre-bebé amplía el objetivo de atención materno-infantil y permite comprobar el aumento de cuidado con el bebé por medio del lenguaje de articulación cuerpo/lenguaje.
